Most concerns between a client and a therapist can be resolved directly, but some situations may require additional support. In accordance with House Bill 4224 and the Texas Health and Safety Code Section §181.105, the following information is provided should you wish to request access to your health care records or file a complaint.
You have a right to have your concerns heard and resolved in a timely manner. Please always feel free to reach out to provide feedback.
Clients have the right to request access to their health care records and providers are required to retain your records for 7 years.
Texas law requires that requests for mental health records be made in writing. To request your records:
- Submit a written request by email or via the portal. Please include your full name, date of birthday, and the specific records you are requesting.
- Requests may be subject to identity verification to protect client privacy and confidentiality in accordance with applicable state and federal laws. Requests are subject to applicable fees as permitted by law.
- Records will be provided within the timeframe required by Texas law.
If you believe your rights have been violated or you wish to file a consumer complaint, you may contact the Office of the Texas Attorney General’s Consumer Protection Division.
